The Social Worker Case Manager - Rehab assists in overseeing and directing patient care activities of a specific caseload of patients through the coordination of care with patient care team personnel, physicians, and other disciplines required to ensure that patient outcomes are achieved within an established time frame. Develops plan of care for the patient's rehabilitation program and oversees the direct delivery of care for assigned patients by patient care team. The following are general characteristics of this job, although duties may vary.
Bachelor’s Degree in social work
2 years of related work experience necessary in order to gain full knowledge of federal, state and local regulations related to discharge planning and knowledge of health and welfare resources and facilities
LSW Or LCSW. Shall comply with current state licensing and certification requirements or jurisdiction(s) in which practicing
Social Worker with a BSW who has not obtained 3 years of supervised clinical hours will be given the opportunity to complete this clinical practicum required to sit for the examination for practice of social work. All BSW Mission Partners must obtain applicable license as awarded by the Department of Professional Regulation within current state of practice within 5 ½ years from date of hire with detailed timeline below.
Mission Partner is required to identify clinical supervisor by day 91 of hire with a plan to complete the supervision within in 3 years
Mission Partner is accountable to ensure completion of state application form
Once the 3 year clinical supervision is completed the Mission Partner must submit state application within one month
Mission Partner must sit for the licensing exam within one year of application submission and pass the exam within 2 years
All BSW Mission Partners must obtain licensure within 5 ½ years from date of hire
If a Mission Partner comes in with documented supervised clinical hours those will be deducted from the 5 ½ year time requirement
Licensed Clinical Professional Counselor (LCPC) license also considered
Care Management Certification, as appropriate, for adult or pediatric within 3 years of hire
CPR certification – American Heart Association within 90 days of hire
Demonstrate the knowledge and skills necessary to provide patient care that is appropriate to the age of the patients served on their assigned unit(s). This requires that the incumbent demonstrates knowledge of growth and development as well as the physical, emotional, and psychosocial needs of the patient population served
Knowledge of rehabilitation therapies and techniques of casework processes plus principles of public welfare
Interpersonal skills necessary in order to obtain information from patients and families to provide counseling and interact effectively with patients, families, Medical Center staff and physicians
Analytical skills necessary to assess patients' needs, develop associated discharge planning and provide sound advice and guidance
Data management skills required for tracking databases, such as for multiservice patient data and insurance benefit tracking
