OSF HealthCare APN Fellowship in Primary Care is a 12 month post graduate
program with immersive training in primary care (internal medicine, family
medicine, and pediatrics) with a focus on the rural and medically underserved
populations. The program will allow for faculty-proctored, team-based learning
in a variety of primary care and community settings, with regular rotations to
specialty medicine practices, digital medicine, behavioral health, and urgent
care. Acceptable candidates will be licensed in Illinois and be able to provide
outpatient care to both adult and pediatric patients. Weekly travel to rural
and medically underserved populations is a priority for this opportunity.
In accordance with professional standards of care and adhering to corporate
philosophy of OSFMG, the advanced practice nurse/
shall be responsible for:
Providing high quality patient care to the patients of OSF.
Providing such professional Advanced Practice services.
Maintaining the consistency and quality of all services provided
to the Practice and its patients.
Maintaining full and current patient medical records, including
reports of all
examinations, procedures, and other services performed by
Provider or other support staff.
Exercising diligence in assisting the Ministry CEO in keeping
costs of the
Practice to a minimum while maintaining a high level of quality
care for patients.
Providing a truly personalized, patient-centric experience to
patients of the
Practice and the OSF Hospital aligned to the patient's
preferences, goals and needs.
Demonstrates the knowledge and skills necessary to provide patient
care that is appropriate to the ages of the patients served.
Licensed to provide advanced practice nursing services in the
State of Illinois as issued by the Illinois Department of Financial and
Professional Regulation in accordance with the Illinois Nurse Practice Act
