Description

The Social Worker Case Manager Inpatient is responsible for identifying, assessing, planning and coordinating, implementing, monitoring and evaluating options and services across the continuum of care for individuals in assigned case load. Health care services are coordinated on behalf of the patient to promote the delivery of care at the appropriate level of care which will optimize clinical outcomes and satisfaction with services. The Case Manager assures the designation of primary responsibility among team members to each aspect of the plan, avoiding duplication and fragmentation. Coordinates with the health care team to facilitate and expedite the best outcomes for the patient and physician caring for that patient, establishing goals and realize outcomes as determined by current individualized care plans. Assesses and intervenes to address the complex psychosocial and behavioral health issues and needs associated with hospitalizations and transition plans, demonstrating the knowledge and skills necessary to provide services to the assigned patient population. Works with the Care Management Division leadership team to ensure alignment with the strategic goals and visions. Requires flexible work schedule, including weekend, holiday, on-call rotation.

Qualifications

REQUIRED QUALIFICATIONS:

 

Bachelors  

 

SOC-Social Work - Level 1 

 

LSW, or LCSW, or LCPC. Shall comply with current state licensing and certification requirements or jurisdiction(s) in which practicing.

 

 

Social Worker with a BSW who has not obtained 3 years of supervised clinical hours will be given the opportunity to complete this clinical practicum required to sit for the examination for practice of social work. Must obtain applicable license as awarded by the

Department of Professional Regulation within current state of practice within 5 ½ years from date of hire with detailed timeline below. 

 

 

Identify clinical supervisor by day 91 of hire with a plan to complete the supervision within in 3 years

 

 

Accountable to ensure completion of state application form

 

 

Once the 3 year clinical supervision is completed, must submit state application within one month

 

 

Must sit for the licensing exam within one year of application submission and pass the exam within 2 years

 

 

Must obtain licensure within 5 ½ years from date of hire

 

 

If there is documented supervised clinical hours, those will be deducted from the 5 ½ year time requirement

 

 

 

Care Management Certification, as appropriate, for adult or pediatric within 3 years of hire.  If certification exam is not meet within 3 years, the deadline can be extended by one year.

 

 

CPR certification within 90 days of hire.

 

 

3 years of experience caring for patients with progressively increasing responsibility

 

 

 

 

PREFERRED QUALIFICATIONS:

 

3 years of experience caring for patients with progressively increasing responsibility

Chronic, complex illnesses adult and/or pediatric care management experience
