The Virtual Care Registered Nurse is a member of a collaborative team who is responsible for providing virtual nursing care to a diverse population of critically, chronically ill adults across the Ministry through technology in their (the patient's) home environment. The nurse is located in Peoria, IL. Provides proactive monitoring, prevention, early intervention and ongoing care with the goal to optimize quality of life for the patient at home through access to healthcare. Utilizes evidenced-based care and clinical decision support to guide practice. Performs ongoing communication and works to promote quality of care through collaboration with physician, advanced care providers and all service team members, patients and families. Acts as a patient advocate and resource to other health care providers. The RN demonstrates service excellence and accountability through recognizing the direct relationship between cost-effective and efficient provision of care and services including the impact of access to care.
OSF Healthcare Nursing Division requires that all employees beginning employment as a Registered Nurse on or after October 1, 2015 are required to earn a BSN degree within six years from Date of Hire and proof of enrollment within your first 18 months of employment
3 years of recent RN experience in adult population
1 year in a role which required effectively communicating and collaborating with health care professionals
Current state RN license
Working knowledge of Windows based computer applications, electronic EMR, and communication platforms
Bachelor’s Degree in Nursing
