Description.Reporting to the Regional Manager of Care Management is responsible for effectively providing daily leadership for Care Management functions at Saint Francis Medical Center. Responsible for assessment, developing and implementing the care processes for care coordination, transition of care and discharge planning in collaboration with frontline staff, team leads, facility leaders, and Regional Care Management leadership. Responsible for ensuring the services and programs provided by the staff achieve established process and outcome measures. Responsible for consistent application and follow through with established policy and procedure around Case/Care Management functions as outlined by Ministry Care Management. Utilizes leadership skills and competencies to create and enhance a culture that supports the Care Management. In accordance with strategic initiatives, incorporates effective change management philosophies in all Performance Improvement efforts. Focuses on consistent performance of staff, supports a proactive approach to outlier management, and escalation of barriers. Manages to the Ministry Care Management vision as it relates to entity case management & social services, ensuring that the below-referenced competencies are delivered in a manner that meets set outcome goals and supports the OSF HealthCare mission and philosophy.
Approximately 3 to 5 years progressively more responsible leadership related work experience necessary in order to gain comprehensive knowledge and expertise needed to develop a strategic perspective and independently solve complex process problems in a collaborative manner in a rapidly changing healthcare environment.
Knowledge and understanding of nursing theory and practice.
Knowledge of healthcare business principles and practices, and an in-depth knowledge of performance improvement and change management methodologies including statistical and financial analysis at a level normally acquired through completion of a Master’s degree or obtain a Master’s degree in Nursing within 5 years.
Interpersonal skills necessary in order to communicate effectively with physicians, Medical Center staff, and regulatory personnel, and lead work of subordinates.
Analytical skill necessary to develop, implement and administer policies, procedures, analyze financial and clinical data and prepare clear, concise reports that identify opportunities and propose appropriate action plans and budgets.
Interpersonal skills in order to coach, mentor and effectively lead assigned personnel, facilitate/organize/direct the work of others and to maintain positive relationships among leadership, ministry/facility staff and internal/external stakeholders.
Must possess proven skills in written/verbal communications, problem solving, independent problem solving, decision making abilities, creativity, prioritization/delegation and necessary computer-related skill.
Working knowledge of an EMR
Minimum education; Bachelor’s of Science in Nursing (BSN) degree required or Master’s in Social Work from a school or program accredited by the Council on Social Work Education.
Registered Nurse with an Associate’s degree will be required to complete a BS within 5 years of hire date.
Current State license as a Registered Nurse.
If MSW, shall comply with the Illinois or Michigan state licensing and certification requirements or jurisdiction(s) in which she or he practices.
Social Worker with an MSW who has not obtained their license will be required to pass the examination for the practice of social work and obtain applicable license as awarded by the Department of Professional Regulation within the employee’s designated state (Illinois/Michigan) of practice. Within 1 year of completing specified supervised clinical hours, the employee will be required to pass the examination for the practice of social work and obtain applicable license as awarded by the Department of Professional Regulation within the employee’s designated state (Illinois/Michigan) of practice.
Care Management experience preferred.
