The Advanced Practice Provider (APRN or PA) will be a member of a collaborative team who is responsible for providing digital care to a diverse population across our Ministry.  Will function across digital care in a variety of services. Utilizes evidenced based care and clinical decision support to guide practice. Performs ongoing communication and works to promote quality of care through collaboration with physician, advanced care providers and all service team members, patients and families. The APP demonstrates service excellence and accountability through recognizing the direct relationship between cost-effective and efficient provision of care and services including the impact of access to care.


 

Primary Job Responsibilities and Accountabilities

 

·        Responsible to work across various digital health services to provide care to patients.

·        Demonstrates the ability to lead other team members and is capable of setting priorities.

·        Communicates with other patient care providers including physicians, other advanced practice providers, residents, medical students, RNs, etc. to exchange information on condition of the patient, discuss progress (or lack of progress) and serves as resource to the same regarding care of the patient.

·        Collects and analyzes patient and family data for the purpose of assessment, diagnosis and management of care.

·        Efficiently assembles a focused assessment of physiological, psychological, and medical record data.

·        Uses critical thinking skills in diagnosing clinical problems and in defining interventions and uses credible clinical decision support tools in determining evidence-based practices for clinical problems.

·        Responds to routine, urgent and emergent patient problems appropriately by demonstrating effective decision making. Provides follow up to ensure problem resolution. Prioritizes clinical problems accurately and in a timely manner

·        Focuses on patient experience/wellbeing and advocates needs and wishes of patients and families with regard to health care.

·        Protects patient privacy and confidentiality by adhering to Health Insurance Portability and Accountability Act (HIPAA) regulations.

·        Learns basic and advanced features of the various telehealth platforms to enhance efficiency in assessment and documentation. Learns and updates word processing, email and web based skills to enhance communication and educational growth.

·        Participates in regularly scheduled staff meetings.

Qualifications


Required:


·        Education:


o   Master’s Degree in Nursing or Physician Assistant studies with board certification


·        Experience –    


o   Minimum of 3-5 years of clinical experience


 


Other skills/knowledge:


·        Excellent communication, organizational, and interpersonal skills


  • Understanding of the principles of performance improvement, case management, discharge planning, utilization review, and medical records
  • Understands the basics workings of Joint Commission, DRG’s, and Medicare, Medicaid, and managed care reimbursement.
  • Has a proven record of teamwork
  • Ability to work independently
  • Ability to work variable shifts
