Job Description

Description

Telephone triage registered nurses are often the first point of access to care for patients from conception through all phases of life, including infancy, childhood, adulthood and geriatrics. The telephone triage nurse demonstrates the knowledge necessary to provide episodic patient care that includes comprehensive symptom assessment, patient education, preventative care, acute psychosocial support, and timely individualized clinical recommendations for after-hours care. Critical thinking and excellent communication skills are required for this unique nursing specialty that interacts with multiple outpatient providers, including primary care, obstetrics, and surgery. The superior initial orientation and continuing education strengthen nurses’ assessment and communication skills that enable them to establish trust with patients quickly, identify acute health care needs, and develop a plan of care. The nurse demonstrates service excellence and accountability through recognizing the direct relationship between cost-effective and efficient provision of care and services including the impact of post discharge calls on 30 day readmission rates. The diverse group of Mission Partners enjoy a positive work environment that includes autonomous and collaborative care within the multidisciplinary team, a supportive work team, and flexible scheduling that accommodates nurses for work-life balance. Triage nursing practice follows the mission of the Sisters of the Third Order of Saint Francis, servicing the community with the ‘greatest care and love.’
