Provider is responsible for providing [advanced practice nursing/physician assistant] services in the OSF Hospital, office setting, and/or urgent care setting (the "Practice"). A written annual summary will be developed based on the following criteria:
1. Clinical practice of [advanced practice nursing/physician assistant] services.
2. Personal traits/attitude/perceptions of Provider.
3. Provider's performance of principal duties and responsibilities set forth below.
This evaluation will be completed yearly and made part of the Provider's permanent file.
Licensed to provide [advanced practice nursing services in the State of Illinois as issued by the Illinois Department of Financial and Professional Regulation in accordance with the Illinois Nurse Practice Act] OR [physician assistant services as issued by the Illinois Department of Financial and Professional Regulation in accordance with the Illinois Physician Assistant Practice Act of 1987] ("Act").
Licensed by the Federal Drug Enforcement Administration and State of Illinois for prescribing of narcotics.
