Weekend shift. Saturday and Sunday.

The Social Worker Case Manager (SW Case Manager) is responsible for identifying, assessing, planning and coordinating, implementing, monitoring and evaluating options and services across the continuum of care for individuals in assigned caseload in an inpatient setting. The SW Case Manager assures the designation of primary responsibility among team members to each aspect of the plan, avoiding duplication and fragmentation. Assesses and intervenes to address the complex psychosocial and behavioral health issues and needs associated with hospitalizations and transition plans. The SW Case Manager coordinates with the health care team to facilitate and expedite the best outcomes and satisfaction with services for the patient and physician caring for that patient. The SW Case Manager works with the patient and treatment team to help establish goals and realize outcomes as determined by current individualized care plans and works with the Care Management Division leadership team to ensure alignment with the strategic goals and visions.

Qualifications

REQUIRED QUALIFICATIONS:

 

Bachelor’s degree in Social Work (BSW)

 

 

3 years of experience caring for patients

 

 

Broad knowledge of chronic illness and medical and community resources.

 

Positive, empathetic approach and ability to communicate effectively to patients/families with a variety of health issues, age groups and socioeconomic backgrounds.

 

Working knowledge of a clinical EMR system and Microsoft Office applications.

 

Flexible work schedule—weekend, holiday, and on-call rotation. Requires the ability to successfully use independent judgement and make decision and requires little supervision.

 

Work requires substantial interpersonal skills to develop and maintain cooperative working relationships with personnel form other departments, with excellent time management skills.

 

 

LSW Or LCSW. Shall comply with current state licensing and certification requirements or jurisdiction(s) in which practicing.

 

 

Social Worker with a BSW who has not obtained 3 years of supervised clinical hours will be given the opportunity to complete this clinical practicum required to sit for the examination for practice of social work. All BSW Mission Partners must obtain applicable license as awarded by the Department of Professional Regulation within current state of practice within 5 ½ years from date of hire with detailed timeline below. 

 

Mission Partner is required to identify clinical supervisor by day 91 of hire with a plan to complete the supervision within in 3 years

 

Accountable to ensure completion of state application form

 

Must submit state application within one month once the 3 year clinical supervision is completed

 

Must sit for the licensing exam within one year of application submission and pass the exam within 2 years

 

Must obtain licensure within 5 ½ years from date of hire

 

If a Mission Partner comes in with documented supervised clinical hours those will be deducted from the 5 ½ year time requirement

 

 

Social Worker with an MSW who has not obtained their license will be required to pass the examination for the practice of social work and obtain applicable license as awarded by the Department of Professional Regulation within the current state of practice within 2 ½ years from date of hire with detailed timeline below.

 

Must start licensing application process by day 91 of hire

 

Must sit for the licensing exam within one year of application submission and pass the exam within 2 years

 

Must obtain licensure within 2 ½ years from date of hire

 

 

Licensed Clinical Professional Counselor (LCPC) license also considered.

 

Care Management Certification, as appropriate, for adult or pediatric within 3 years of hire.

 

CPR certification by the American Heart Association within 90 days of hire.

 

 

 

PREFERRED QUALIFICATIONS:

 

Master’s degree in social work (MSW)

 

 

Experience with chronic, complex illnesses adult and/or pediatric care management
