Description

The Social Worker II shares the responsibility of identifying social and emotional needs of each client. The position assists with arranging services to meet the needs of residents, providing liaison between the client, family, facility staff, and the community. Assess all patients admitted to the Mental Health department, gathers information from available sources including family, agencies, friends and others, analyzes the information, formulates treatment recommendations, communicates pertinent information to other members of the interdisciplinary treatment team, participates in the development of the patient's treatment plan, coordinates or provides services including individual, group, family, marital and/or group psychotherapy and discharge planning. Serves as a resource to staff and community regarding psychosocial issues. Participates in Hospital and departmental planning committees and teams.

Qualifications

REQUIRED QUALIFICATIONS:

 

Master’s degree in Social Work from a school approved by the Council on Social Work Education or related field.

 

 

Minimum 2 years of hospital social work, relevant field work placement in the mental health field or home care experience is preferred.

Proficient in Microsoft Office software (Word and Excel).

 

 

Current State of Illinois license as a Licensed Social Worker (LSW), or as a Licensed Clinical Social Worker (LCSW) required. Current Cardiopulmonary Resuscitation Certification per department policy.

 

Valid Driver's License.

 

 

 

Crisis Prevention Institute Certification required within 90 days of hire.
