Eligible for $5,000 sign on bonus with 2 year commitment.
The Physical Therapist at OSF Home Care demonstrates the knowledge and skills necessary to provide patient care that is appropriate to the ages of the patients served. Coordinates inter-disciplinary care to patients, which may include nursing, occupational therapy, speech therapy, social work, home care aide, home medical equipment and home infusion services as needed. Formulates progressive care plans to restore function and prevent disability following illness or injury. Provide patient-centered care, using critical thinking skills to ensure positive patient outcomes. The PT is responsible for care coordination, appropriate visit pattern utilization, outcome monitoring/management, resource management, episode management and supervising of physical therapist assistants.
Allows flexibility in their schedule to accommodate patient needs.
This position will work 5 out of 6 Saturday or Sunday.
Qualifications
REQUIRED QUALIFICATIONS:
Bachelor’s, Master’s, or Doctorate degree in Physical Therapy.
Requires current state license to practice as a Physical Therapist.
Current American Heart Association HealthCare Provider BLS is required prior to start date of employment.
Proficient with electronic documentation.
Provides own transportation with auto insurance and a valid driver’s license.
Abides by all laws, rules, and recommendations to comply with OSF Safe Driving Policies while on duty.
Must be able to work independently in virtual work environment and execute own workload.
PREFERRED QUALIFICATIONS:
1 year recent physical therapy experience.
Home Health experience.
Experience with Microsoft Outlook, Word, Excel.
Knowledge of the PT State Practice Act and Home Care rules and regulations.
