Description

The Hospice Registered Nurse provides patient centered end-of life care in the person’s home environment. The Hospice Registered Nurse is responsible for a scope of care that encompasses all ages from infancy through geriatric. The Hospice Registered Nurse demonstrates the knowledge and skills necessary to provide patient care that is appropriate to the ages of the patient served. The Hospice Registered Nurse implements the plan of care and coordinates care with other disciplines such as social work, home care aide, volunteers, chaplain, advance nurse provider, physician and medical director with a focus on quality of life, symptom management, comfort and support for both the patient and their family. The Hospice Registered Nurse is responsible for care coordination, appropriate visit pattern utilization, outcome monitoring/management, and resource management. The Hospice Registered Nurse allows flexibility in their schedule to accommodate patient needs.

Qualifications

REQUIRED QUALIFICATIONS:

 

Current RN license through the State of Illinois


OSF HealthCare Nursing Division requires that all employees beginning employment as a Registered Nurse on or after October 1, 2015 are required to earn a BSN degree within 6 years from the date of hire and proof of enrollment within your first 18 months of employment

 

1 year of nursing in a clinical setting

 

Driver’s license and auto insurance

 

Current CPR through AHA

    

Knowledge of state, federal and JCAHO regulations

 

Knowledge of documentation of electronic medical records

 

Ability to work independently and execute own work load

 

 

 

 

 

PREFERRED QUALIFICATIONS:

 

Home health or hospice experience

 

National certification for Hospice and Palliative care

 

Knowledge of EPIC
