Description

Acts as a liaison for OSF Ministry Care Management staff. Assists Care Management staff by arranging, documenting, and following-up on post-acute and transportation needs. Responsible for appropriate use of resources to ensure our patients receive care that is continuous, comprehensive, and coordinated across the care continuum. The Service and Resource Specialist collaborates with case managers and post-acute service representatives to determine the most appropriate post-acute services to meet the needs of the patient and ensure a safe transition from acute level of care. The Service and Resource Specialist will ensure communication is appropriate, accurate and timely.

Qualifications

REQUIRED QUALIFICATIONS:

 

Associate’s degree or equivalent experience.

 

 

2 years of healthcare or pertinent experience.

 

 

Demonstrated ability to work independently and execute own workload. 

 

Requires flexible work schedule, including weekends and holidays. 

 

Flexibility and willingness to adapt to evolving job roles within the constantly changing work environment. 

 

Proven ability to think critically, work independently, prioritize own workload to ensure tasks are completed in a timely manner and within established timeframes. 

 

Must be detail oriented, accurate, and organized. 

 

Requires excellent interpersonal skills necessary to answer telephones, communicate with team members, department staff, and external personnel, exchange and gather accurate information on factual matters, and perform other activities requiring courtesy and tact to continue continuity of care.

 

 

PREFERRED QUALIFICATIONS:

 

Medical terminology background.
