Description

The Virtual Advanced Care Center (VAC) Registered Nurse is a member of a collaborative team who is responsible for providing virtual nursing care to a diverse population of critically, chronically ill adults across the Ministry through technology in their home environment. Provides proactive monitoring, prevention, early intervention and ongoing care with the goal to optimize quality of life for the patient at home through access to healthcare. Utilizes evidenced based care and clinical decision support to guide practice. Performs ongoing communication and works to promote quality of care through collaboration with physician, advanced care providers and all service team members, patients and families.  Acts as a patient advocate and resource to other health care providers such as Home Health and Outpatient Palliative Care. The RN demonstrates service excellence and accountability through recognizing the direct relationship between cost-effective and efficient provision of care and services including the impact of access to care. Qualifications

REQUIRED QUALIFICATIONS:

 

OSF Healthcare Nursing Division requires that all employees beginning employment as a Registered Nurse on or after October 1, 2015 are required to earn a BSN degree within six years from Date of Hire and proof of enrollment within your first 18 months of employment.

 

Current RN Illinois license

 

3 years of recent RN experience in adult population.

 

1 year in a role which required effectively communicating and collaborating with health care professionals

 

Proficient with Windows based computer applications

 

Proficiency with electronic EMR and communication platforms

 

PREFERRED QUALIFICATIONS:

 

Bachelor Degree in Nursing

 

Home Health nursing experience
