Job Description


Reports to the Supervisor of Complex Care Management. Serves as a key member of the Complex Care Management team and Primary Care team to promote disease management, preventative care and wellness. Promotes, encourages and engages them the patient to take an active role in their health by providing them with tools necessary to make healthy life style choices and adopt life-long healthy behaviors. In collaboration with the Complex Care Manager coordinates, monitors, and follows up with patient to ensure patient they are working toward the comprehensive plan of care and care goals. Performs ongoing communication and works to promote quality of care through collaboration with the Complex Care Manager and all service team members, patients and families.




Associate’s degree in health education, health promotion or equivalent.


Two years of experience in clinical, counseling or health care setting caring for patients with chronic, complex illnesses (adult experience for adult and pediatric experience for pediatric) adult and/or pediatric care management experience preferred.   Broad knowledge of chronic illness and medical and community resources required.


Positive, empathetic approach and ability to communicate effectively to patients/ families with a variety of health issues, age groups and socioeconomic backgrounds. 


Wellness and coaching experience preferred.


Flexible work schedule – may need to work evenings/occasional weekends to provide patient access and/or follow up.


Requires the ability to successfully use independent judgment and make decisions and requires little supervision.


Current Illinois and/or Michigan LPN license.


Must possess strong independent problem solving skills.


CPR Certification within 90 days of hire.


Working knowledge of a clinical EMR system and Microsoft Office applications.


Able to work with other and efficiently with excellent time management skills.


Work requires substantial interpersonal skills to develop and maintain cooperative working relationships with personnel from other departments.
