Description

The Home Transition Specialist is the primary resource engaging in supporting patients in need of post-acute services as they transition from an acute setting to the home.  They collaborate with hospital providers and case managers to determine the most appropriate home care services for the patient and assist in transitioning them home with the identified service upon discharge.  The Home Transition Specialist will evaluate patient needs of home care services, participate in discussion about patient/family needs, and progress in discharge preparation. The Home Transition Specialist will meet with patients and their families in person or telephonically to discuss services, provide education regarding home services and equipment and offer guidance to families regarding Home Care preparations. The Home Transition Specialist will ensure internal communication is appropriate, accurate and timely as well as ensure communication with patients and families is timely, consistent, and includes appropriate expectation setting and teach back. 

Qualifications

REQUIRED QUALIFICATIONS:

 

Associate’s degree or equivalent experience in a healthcare setting necessary to develop mastery of the required skills and to gain a thorough understanding of department/unit policies, procedures and operations to assume duties of interacting with patients/families, and other department staff.

 

 

1 year of home care or pertinent experience

 

 

Demonstrates the ability to work independently and execute own workload. 

 

Requires critical thinking ability necessary to prioritize own workload to ensure tasks are completed in a timely manner and within established timeframe.

 

Interpersonal skills necessary to answer telephones, communicate with team members, department staff, and external personnel, exchange information on factual matters, interact with patients/families and perform other activities requiring courtesy and tact. 

 

Interact with members of the health care team and external personnel in order to exchange and gather patient care data to continue continuity of care.

 

 

Must have or obtain within the first 90 days of employment certification in CPR.

 

 

 

PREFERRED QUALIFICATIONS:

 

Working knowledge of computer software applications and electronic health record.
