Job Description


The Virtual Advanced Care Center (VAC) Advanced Practice Nurse (APN) is a member of a collaborative team who is responsible for providing virtual care to a diverse population of critically, chronically ill adults across the Ministry through technology in their home environment. According to the professional nursing standards of care, performs a variety of advanced nursing for patients in the VAC through expert education and consultation. Provides proactive monitoring, prevention, early intervention and ongoing care with the goal to optimize quality of life for the patient at home through access to healthcare. Utilizes evidenced based care and clinical decision support to guide practice. Provides nursing care according to physician's orders and in conformance with the ANA Standards of Practice and Care, ANA Code of Ethics, Unit specific Standards, Patient Services policies and procedures, unit specific policies and procedures, and the Illinois Nurse and Advanced Practice Act. Performs ongoing communication and works to promote quality of care through collaboration with physician, advanced care providers and all service team members, patients and families. The APN demonstrates service excellence and accountability through recognizing the direct relationship between cost-effective and efficient provision of care and services including the impact of access to care. Master’s Degree in Nursing


3 years of clinical experience


National Certification within 3 years of hire


IL license as a Registered Nurse


IL Licensure as an Advanced Practice Nurse


IL Advanced Practice Nurse controlled substance license


Controlled substance registration certificate


Excellent communication.  Excellent telephone etiquette and customer service through effective communication skills with all calls.


Understanding of the principles of performance improvement, case management, discharge planning, utilization review, and medical records


Understands the basics workings of Joint Commission, DRG’s, and Medicare, Medicaid, and managed care reimbursement.


Ability to work independently.


Proficiency with Windows based computer operations.


Proficient in utilizing an electronic EMR system. 





National Certification preferred (Hospice, Palliative Care)


Experience with critically, chronically ill patients and families




Working knowledge of Epic EMR System
