Description

The Patient Navigator leads the day-to-day efforts of welcoming new patients to the OSF Physician Enterprise, which includes aligning patients with the right provider in the right location and scheduling them at the right time.  In addition, the Patient Navigator acts as a resource to patients and departments to provide guidance for services that support population health management initiatives.  This includes, but is not limited to, ACO and HMOI outreach for the purpose of re-engaging patients with their provider care teams for optimal quality and financial outcomes.  The Patient Navigator is also responsible for performing patient retention outreach to re-align patients with another primary care provider when their provider leaves OSFMG.  This position is responsible for creating an exceptional telephone and/or electronic virtual service for patients wishing to establish with an OSF PCP or Specialty Office. Understands the connection of patient experience with patient loyalty and integrates service excellence with each patient interaction.  Maintains daily awareness of provider access throughout the ministry, and identifies availability for new patients to establish.  Identifies, escalates and works to resolve barriers to access.  Performs a variety of duties including answering phone calls, scheduling, registration, transferring phone calls, and providing assistance to patience using OSF protocols.  Identifies patient concerns and involves the appropriate people and resources for a quick resolution.  In coordination with the manager, leads the service excellence efforts with peers.  Provides exceptional customer service and collaborates with other departments to provide an exceptional patient experience.

 

 

Qualifications

REQUIRED QUALIFICATIONS:

 

Two years of work experience in a medical office setting is required.

 

MOA or CMA is required.

 

Demonstrates service excellence skills during all patient interactions. 

 

Demonstrates high communication skills with professional voice and tone. 

 

Possesses problem solving skills.

 

The incumbent should have patient advocacy skills, basic computer keyboarding skills including typing 35 words a minute; self reliant and independent decision making skills, attention to detail, and the ability to work in a team environment.

 

 

 

 

PREFERRED QUALIFICATIONS:

 

Completion of a 2 year degree is preferred.

 

3 years of experience in a customer service role with heavy phone work preferred.
