Provides comprehensive social work services to patients, families and donors through all phases of transplantation or donation. As a member of the transplant team, the social worker provides patient education and emotional support, conducts interventions, is responsible for referrals to community agencies, and assists with the coordination of patient care. Assesses the patient’s physical, psychosocial, cultural and spiritual needs. Assists in the coordination of appropriate services, resources and referrals.
Master’s Degree in Social Work from an accredited school of social work
2 years of social work experience in a hospital or health care setting
Current state of Illinois license in social work (LSW)
Strong assessment skills and analytical ability
Excellent interpersonal skills, strong verbal and written communication skills, and basic computer skills
Certification as a Clinical Transplant Social Worker
