Description

The Oncology Navigator functions on a multidisciplinary team, acting as an advocate, an educator and a coordinator of care for Oncology patients, to eliminate barriers to timely care, facilitate flow through the system, and increase patient and provider satisfaction. Navigator will establish early contact with the cancer patients to identify and eliminate barriers to timely diagnosis and treatment. Will assess clinical, emotional, spiritual, psychosocial, financial and other patient needs. This person works with a multidisciplinary team to provide improved comprehensive oncology services to patients and assists in training and education of oncology staff. The Navigator will be responsible to monitor the patient’s survivorship care plan and report on outcome measures. Will be responsible to work with secretarial staff to schedule tests, procedures, appointments and treatments. Will participate in multi-disciplinary consultations and/or tumor conferences. Navigator will refer patients to appropriate supportive services and community resources. Serve as a clinical resource with expertise in oncology care management in reinforcing education with patients & families, in addition to providing input on educational materials.

Qualifications

REQUIRED QUALIFICATIONS:

 

Graduate of an accredited school of nursing.

 

Michigan RN License

 

3 years oncology patient care experience as an RN within the last 5 years

 

ONS Chemotherapy Biotherapy Certificate Course and/or OCN certification

 

Current certification by the American Heart Association Healthcare Provider BLS

 

BSN (mandatory within 6 years of hire)
