DescriptionThe Home Care Aide is under the direction of a Registered Nurse or Licensed Physical Therapist/Occupational Therapist and in accordance with a plan of treatment to provide personal care and emotional comfort. To promote the maintenance and rehabilitation, if applicable, of the patient and to facilitate a safe home environment. Demonstrates the knowledge and skills necessary to provide patient care that is appropriate to the ages of the patients served. This requires that the individual demonstrates knowledge of the principles of growth and development as well as the physical, emotional and psycho-social needs of the patient population served.
High School diploma or GED.
Completion of a Home Care Aide training program which meets the Federal requirements for competency.
State license as a certified nurse aide (CNA).
Must have certification in CPR prior to patient contact.
Demonstrates ability to work independently and execute own workload.
Must be able to use and operate medical and/or office equipment, including Point of Care devices.
Demonstrates the knowledge and skills necessary to provide patient care appropriate to the age of the patients served.
Demonstrates strong oral and written communication skills.
Able to carry out instructions & execute assignments independently.
Accountability to the Hospice Home Manager.
